Preschool children who undergo hospitalization often experience anxiety due to an unfamiliar hospital environment, medical procedures, and separation from their families. Anxiety that is not well managed can hinder the healing process and reduce children’s cooperation during treatment. This study aimed to determine the effectiveness of educational play therapy in reducing anxiety levels among preschool children during hospitalization at Madani Regional General Hospital, Central Sulawesi Province. This scientific work employed a case study design with a nursing care approach. The sample consisted of five preschool children aged 3–6 years who were hospitalized and experienced anxiety. Anxiety levels were measured using the Preschool Anxiety Scale (PAS) before and after the intervention. Educational play therapy was provided through storytelling, drawing, role-playing, and other educational play activities. Data analysis was conducted descriptively by comparing the mean PAS scores before and after the intervention. The results showed a decrease in anxiety levels following the implementation of educational play therapy. The mean PAS score before the intervention was 42.6 and decreased to 28.4 after the intervention. This reduction indicated a change in anxiety category from moderate–severe to mild. Clinically, the children appeared calmer, more cooperative, and better able to adapt to the hospital environment. In conclusion, educational play therapy is effective in reducing anxiety levels among preschool children during hospitalization. Therefore, educational play therapy is recommended to be integrated as a routine non-pharmacological intervention in pediatric nursing care.
