Low self-concept, lack of self-confidence, loss of motivation, decreased moral control, and inability to
fulfill spiritual needs are precipitating factors for schizophrenia. Chronic low self-esteem can lead to social
isolation, hallucinations, and an increased risk of violent behavior. Physical activity is known to enhance
physiological adaptation, including the release of neurotransmitters such as serotonin, which play a role in
stress regulation and mood improvement. This study aimed to analyze the implementation of physical strength
activity based on evidence-based nursing in patients with chronic low self-esteem in the inpatient ward of UPT
RSUD Madani. This study employed a pre-experimental design using a one-group pre-test–post-test approach
without a control group. The sample consisted of 5 patients selected through purposive sampling. The
intervention, in the form of physical activity exercises, was conducted over 2 days following nursing care
strategies. The results showed a significant improvement after the intervention, indicated by a reduction in signs
and symptoms and an increase in patients’ abilities. It can be concluded that physical activity exercises are
effective in reducing symptoms of chronic low self-esteem. Therefore, this therapy is recommended to be
integrated in a structured and scheduled manner as a complementary therapy in the care of patients with
chronic low self-esteem.
